2012 Spring Conference: Collaborative Management of Esthetic Dilemmas

The American Academy of Periodontology invites you to a conference April 28-29 in Chicago to showcase the relationship between periodontists, orthodontists and restorative dentists, and explore what can be achieved when everyone works together as a team for optimal patient outcomes.

Fees

AAP Member	$550
1-3 sponsored dentists	$315 per person
4+ sponsored dentists	$260 per person
Non-sponsored general dentist	$1,100
Students (after 3/1/12)*	$250

Only one form of payment will be accepted. Registration fee includes: educational sessions, continental breakfast and breaks each day, Saturday lunch, as well as the Welcome Reception. Registered attendees who wish to bring a spouse/guest to the Welcome Reception can purchase a ticket for $35 per person.

*Students will be able to register at a rate of $250 per person beginning March 1, 2012. Documentation of student status must accompany Registration & Housing Form in order to qualify.

Cancellation Policy

Refunds will be granted to registrants who submit written requests prior to April 14, less a set administrative fee based on handling costs:

Prior to March 29, 2012 Full refund, minus $100
March 29 - April 14, 2012 Full refund, minus $200
After April 14, 2012 No refund

Prior to April 5, all housing must be reserved through Experient, and attendees must register for the conference to secure housing; however, multiple room requests per registrant are allowed. Hotel cancellations and requests for changes must be made in writing to Experient at:

CONTINUING EDUCATION

Continuing education (CE) is offered on an hour-for-hour basis for every hour of programming that earns CE credit. 10.5 credits are available for attending this conference. A pre-printed CE form will be included in your credential packet.

Options are available to reach both the downtown area and the Sheraton:

From O’Hare International Airport (Distance: approximately 18 miles, drive time: 45 minutes): Take the CTA’s Blue Line train from the airport to the Grand stop, located 0.6 miles northwest of the hotel (currently $2.25 one way). Cab rides will average $45, with limousine services at $85.
From Midway International Airport (Distance: approximately 13 miles, drive time: 30 minutes): Take the CTA’s Orange Line train from the airport to the Clark/Lake stop, located 1 mile southwest of the hotel (currently $2.25 one way). Cab rides will average $35, with limousine services at $85.
